Step 3: Grant OPSUCA Power of Attorney
Additional Forms: Powers of Attorney
In order for OPSUCA to receive all notifications from the OESC, including claims notices, determinations, and appeal documents and so on, OPSUCA must have two separate Power of Attorney forms completed to receive these documents on your school’s behalf. The forms are available at the following link: https://www.ok.gov/oesc/index.php?c=11. The necessary Power of Attorneys forms are OES 190-B and OES 190-T. OPSUCA will help your school complete this form and any other necessary forms when joining.
